The Patient Care Coordinator is the point of contact to ensure patients' needs are met. The PCC coordinates patients' treatment plans and follow up, order changes, and renewals.

Work Environment & Physical Demands
In Office - 3300 Providence Drive, Suite B04, Anchorage, AK 99508. This role is in a professional office setting with moderate noise level. The position requires regular use of standard office equipment such as computers, phones, photocopiers, filling cabinets, and fax machines. The work environment is fast-paced and may involve frequent interruptions, shifting priorities, and regular interaction with patients, families, referral sources, and clinical staff. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the role.
The position is primarily sedentary and requires the ability to sit for extended periods of time while performing computer-based tasks and telephone communications. The role involved frequent use of hands and fingers to operate computer keyboard, mouse, and other office equipment. The employee must be able to effectively communicate verbally and in writing. Occasional standing, walking, reaching, and light lifting (up to 10 pounds) may be required.
Position Type/Expected Hours of Work
This is a full-time position. Days and hours of work are Monday through Friday, 8:00 AM to 5 PM AKST and on-call.
